This clinical study in 50 infertile men with asthenoteratozoospermia received N-acetylcysteine 600 mg/day orally for 3 months, resulting in significantly increased sperm count and motility while abnormal morphology, DNA fragmentation, and protamine deficiency decreased significantly (P < 0.05), with total antioxidant capacity increasing and malondialdehyde decreasing.

This randomized, controlled study of 120 infertile men with idiopathic infertility showed that N-acetylcysteine 600 mg/day for 3 months had significant improving effects on semen volume, motility, and viscosity, with greater serum total antioxidant capacity and lower total peroxide and oxidative stress index compared to placebo controls.

This double-blind, placebo-controlled, randomized study of 468 infertile men with idiopathic oligo-asthenoteratospermia received 200 μg selenium orally daily (combined with 600 mg N-acetyl-cysteine group) for 26 weeks, resulting in significant improvements in all semen parameters with additive beneficial effects, strong correlation between selenium concentration and sperm concentration, motility, and morphology (r = 0.64-0.67, P = 0.01).

This comprehensive review demonstrated that selenium supplementation improves semen parameters including total sperm count, motility, and survival rate through enhancement of selenoprotein antioxidant biomarkers, with selenium being essential for testicular development, spermatogenesis, and structural integrity of the sperm tail required for optimal motility.

This interventional study demonstrated that combination of 200 mg vitamin C, 200 mg vitamin E, and 400 mg glutathione administered for 2 months in 14 infertile men resulted in significant decreases in oxidative DNA damage (measured by 8-hydroxy-2ʹ-deoxyguanosine assay) and significant improvement in sperm concentration (+7±1 million/mL, P<0.05).

This systematic review of 20 clinical studies found that 19/20 studies showed significant reduction of oxidative stress or DNA damage after oral antioxidant treatment including vitamin C (typically 200-1000 mg daily), with strong evidence supporting improved motility particularly in asthenospermic patients and protection against peroxidative damage and DNA fragmentation.

This experimental study demonstrated that Emblica officinalis (Amla) fruit extract ameliorated reproductive injury and oxidative testicular toxicity induced by organophosphate pesticide exposure in male rats, with amla's bioactive polyphenols (tannins and flavonoids) significantly reducing oxidative stress and ROS levels while enhancing antioxidant enzyme activity.

This functional review demonstrated that Phyllanthus emblica L. (Amla) is rich in antioxidants including gallic acid, ascorbic acid, and phenolic compounds with proven anti-inflammatory, anti-hyperglycemic, and antioxidant activities in both animal and human investigations, with polyphenols significantly reducing oxidative stress and improving male reproductive system health and reproduction.

This clinical trial of 108 fertile and 103 subfertile men randomized into four groups (folic acid only, zinc only, combined folic acid+zinc, or placebo) for 26 weeks showed statistically significant increase of 74% in total normal sperm concentration among the subfertile group receiving combined folic acid and zinc therapy compared to placebo.

This prospective observational study of 125 male subfertile patients treated with folic acid and zinc sulfate showed significant improvements after treatment: rapid linear motility increased from 16.06±14.46 to 22.82±13.40, morphology improved from 19.43±15.48 to 30.18±19.12, demonstrating that micronutrient supplementation with folic acid and zinc causes increase of semen quality and plays a key role in male subfertility treatment.
